Dutch:
On the "Options for All Airlines" tab, I set all my failure percentages to 0.0. For the aircraft I was flying, I had reliability set to 100.0. However, on my last flight I got a +15 for a nice landing with a severe mechanical fault. I didn't notice any faults in flight which seems correct based on my settings, but I would not expect any bonus points either and I did get 15.

Could you please use 'View details' from the Administrator for that flight and give me the fault code number?
Sometimes we have had problems with 'false positives' showing up with the faults.
FSCaptain monitors and detects faults that don't come from it's own triggering system by default. That way if an engine fails because of an airplane fault FSCaptain will recognize that as a failure.
You can turn this off by using the DISABLEIFFD option documented in appendix 4 of the manual.

Thanks! 203 is "partial panel avionics" which is a signal from FS that the avionics have failed. Apparently some airplanes fly with this on all the time. I'm disabling that code in 1.00.02.
