Ehh, I think I know what I'm doing wrong... I switch altimeter to STD 200-300 ft below TA - maybe this is the reason... normally altimeter should be change above TA. However, I didn't check this solution yet...

That should not be a problem. It doesn't care what the altimeter is set below TA. A penalty is only triggered if it's not set to STD (or within a couple of millibars of STD) above TA plus about 1000 extra feet. It's really hard to mess this up if you have the correct TA.
The mostly likely problem is the TA isn't right or isn't what you think it is.

You can actually read this. If our altitude (where the plane is right now) is greater than the TA plus 1000 extra feet then:
Fetch the altimeter setting from FSX, and then check it:
If that setting is more than 29.96 or less than 29.89, and there's no emergency declared, then the penalty is turned on.
That's the deal. There's lots of fudge room here to avoid being picky about it.
There's not much that can go wrong here except if the altimeter setting from FSX is wrong, or the TA is wrong.

Can you confirm that the +1000 is in feet and not in inches? I get underserved altimeter penalties every 10th flight or so, to the point that I am manically obsessed with changing it right at TA. I love FSCaptain, but behind the local time issue this is my number 2 on the minor issues list.

The TransitionAlt is feet, not inches. As you can see from this code, if you TA is 3000 as soon as you cross 4000 it checks for it to be reasonably close to the standard setting. However, our_altitude is obtained directly from the (A:PLANE ALTITUDE,feet) variable in FSX, which the documentation at msdn.microsoft.com/en-us/library/cc526981.aspx defines as "Plane Altitude" with no further explanation. If that is the "Standard altitude" at 29.92 versus the indicated altitude, perhaps that explains how this error could happen. By the way, it never happens to me and I don't have the penalty turned off. I set the altimeter to 29.92 as soon as the FO says "passing transition altitude."

I checked TA in transitions.cfg and in "View release" window in Dispatch step and everything is correct (I mean there are the same values as on aeronautical charts). Anyway, you wrote sentence which revelated me: "I set the altimeter to 29.92 as soon as the FO says "passing transition altitude." (probably thit is because I steel has very quiet FO/FA voices, what I mentioned in another post ) So, I have to use headset or place my speakers near to my head and carefuly listen for FO Ps. FO always says "passing TA" or is this an option?

Ok. However, I have a little enough this situation - yesterday, after departure from EPWA (Poland) I was waiting for FO's words and when he started talk to me (at about 6800 ft [TA is 6500]), I set altimeter to STD. Maybe too fast (?), maybe I should wait when he finish talks (?) ...anyway I had TA penalty again! QNH in Warsaw was 1018.
